Scheme Guidance
A Short Scheme Guide has been prepared as a user friendly guide for members, providing an informative insight into the main provisions and benefits of the Local Government Pension Scheme (LGPS). This guide must be issued to new members within 2 months of joining the scheme, and upon member request.

This guide is published annually and hard copies are distributed to each Employer towards the end of March / beginning of April.

New Member Forms
Upon commencement within the Scheme, a Starter Pack should be issued to a New Member. This pack should contain:
- an Opting In Declaration form
- a Declaration of Previous Pension Rights form
- a Death Grant Expression of Wish form
A member should also be issued a Short Scheme Guide within 2 months of joining.
Please Note
It is essential for each new member to complete a Declaration of Previous Pension Rights form, regardless of whether or not they wish to consider the transfer of any previous pension rights into the LGPS.
These forms have been designed to take account of the information needed by the Fund to administer member records. Please refrain from making any amendments to these documents.

Maintenance Forms
This section contains the forms in relation to your administration of the Scheme.
Medical certificates have also been specifically prepared to gather the information needed from your appointed Independent Registered Medical Practitioner (IRMP) in the event of an ill health retirement.

These forms have been designed to take account of the information needed by the Fund to administer member records. Please refrain from making any amendments to these documents.
If you supply your data via the i-Connect service, please note that there is no need for you to provide a New Member Starter form or Change of Circumstance form (unless you are reporting a period of authorised absence).

Ill Health Certificates
The ill health certificates to be applied from 1 April 2014 have been categorised into the table shown below.
Please Note
It is your responsibility, as the Scheme Employer, to supply the correct Certificate and Guidance to your appointed Independent Registered Medical Practitioner (IRMP).
